guidance is updated, links break and wording can be unclear. If you spot
an error, we want to hear from you.
We welcome corrections from families, providers, professionals,
public agencies and anyone with a reliable source.
How to report a correction
Use the Contact form and choose
Report a correction as the subject. Please include the
page or listing URL, the exact information you believe is wrong or
outdated, the correction you are proposing, a reliable source, link or
document where possible, and your relationship to the information if
relevant.
Do not include medical records, NRIC numbers, full financial details
or other sensitive personal information in the form.
What happens next
We assess reports against available evidence and the purpose of the
page. We may correct a factual error; update a source, date or provider
detail; add clarification or context; temporarily remove or qualify
information while it is checked; decline a change where evidence is
insufficient; or contact the reporter for more information.
We may not be able to disclose the outcome of every report,
especially where privacy or third-party information is involved.
Material corrections will be made as promptly as reasonably
possible.
How we show corrections
For material changes that could affect a reader’s understanding or
decision, we may add a correction note, update note or revised
last-updated date on the page. Routine changes—such as a phone number,
broken link or formatting improvement—may be updated without a separate
notice.
We do not remove accurate information or withhold a correction
because a person or organisation dislikes it. Equally, we do not publish
an allegation, review or demand for change without reliable support.
Provider listing updates
Providers may ask us to update factual listing details. We may
request a source or verification before making the change. An update
request does not create a commercial relationship, and a commercial
relationship does not give a provider control over independent factual
wording.
Appeal or follow-up
If you believe we have misunderstood a correction, send a follow-up
through the Contact form with the original page URL and additional
reliable evidence. We will review the new information.
